The STC-HPS is beginning to provide forms as an interactive PDF you can save.
Simply type in the data that you want to input, save the file onto your
computer, and then attach it to email or print it out to mail it in. It's
really no different than what you would do if you were to print the form,
fill it out by hand, and then scan it onto your computer and attach the file
to the email. The interactive forms simply remove the need to print-out and
complete the form by hand. These files require Adobe Reader. Download the
most current version of Adobe Reader here.
